Zepbound's label, revised 8/2026, lets a single-dose pen or single-dose vial stay unrefrigerated for a total of 21 days at no higher than 30°C (86°F). Multi-dose vials and KwikPens get 30 days unopened. Count the days, because the label counts them for you in total.

Single-dose pens and vials: a total of 21 days

Section 16.2 of the prescribing information revised 8/2026 covers four presentations: the single-dose pen, the single-dose vial, the multi-dose vial and the single-patient-use KwikPen. For the first two, it counts unrefrigerated time as "a total of 21 days" at a temperature no higher than 30°C (86°F).

The word that matters is "total." It is a running count, not a single stretch. Two days in a bag on a weekend and five days on a trip add up to seven.

The label sets no other day count for these two presentations. The label gives one total of 21 days, not a fresh allowance each time the pen leaves the fridge, so one long trip and several short ones draw from the same count.

The label changed, so check the date on what you read

The label from 09/2025 said, for the single-dose pen and vial stored at room temperature, "it should not be returned to the refrigerator." The 8/2026 label replaced that with the total of 21 days.

A page written before 8/2026 can carry the old wording. We are reporting what the current label says and nothing beyond it. If a page and your carton differ, the current label and your pharmacist outrank the page.

Multi-dose vials and KwikPens: 30 days, with three limits once opened

Unopened, the multi-dose vial and the KwikPen can be refrigerated until the expiration date, or kept at room temperature no higher than 30°C (86°F) for 30 days, according to the same section 16.2. After 30 days at room temperature, the label says to discard.

Once opened, the label lists three limits: 30 total days at room temperature, 30 days after first use, and 4 weekly doses. Discard at the first limit you reach, even if medicine remains.

"30 days" alone is the wrong number to remember here, and "21 days" is the wrong number for these two presentations. The label gives each its own rule. For a person injecting weekly, the 4-dose limit arrives before the 30 days after first use (illustrative arithmetic: four weekly doses cover about three weeks).

Freezing and heat

The label's section 16.2 says to protect Zepbound from heat and light, and not to freeze it. It also says not to use medicine that has frozen. A pen that froze is a pen the label says not to use, however recently it left the fridge.

The 21-day and 30-day allowances apply at no higher than 30°C (86°F). A hot car or a sunny windowsill is outside what the label describes, and we do not present any workaround for it. The label gives no count for time above that ceiling.

Keep a count the label can be checked against

Two things to write down

For a single-dose pen or vial: the days it spent out of the fridge, added up. For a KwikPen or multi-dose vial: the date of first use.

Our view: the useful record is a tally, not a memory. "A few days" is how a count of 21 gets exceeded without anyone noticing.

For a single-dose pen, write the date each time it leaves the fridge and the date it goes back, then add the stretches. Illustrative example: out for 2 days on a weekend, back in the fridge, out for 5 days on a trip. The tally reads 7 of 21.

For a KwikPen or multi-dose vial, write the first-use date on the carton and keep it there. Illustrative example: first use on 3 October. Thirty days after first use is 2 November, and the 4-dose limit falls before that for a weekly schedule.

The tradeoff is a few seconds of writing each time the pen moves. That cost is small next to guessing. You cannot read a pen's unrefrigerated days off the pen itself, and a tally is the only way to check the count against the label.

When to ask instead of guessing

If your count is lost, a pen froze, or the carton date is unreadable, ask your pharmacist or prescriber. The site and the dose are yours to choose, with your prescriber. It is fine to stop and talk to them before you inject a pen you doubt.

Questions people ask

Can a Zepbound pen go back in the fridge after sitting out?

The 8/2026 label counts a total of 21 days unrefrigerated for single-dose pens and vials, so time out is added up across stretches. The older 09/2025 label said it should not be returned to the refrigerator. If you are unsure of your count, ask your pharmacist.

How do I count days out of the fridge for a single-dose pen?

Write the date each time the pen leaves the fridge and the date it goes back, then add the stretches. The label's limit is a total of 21 days at no higher than 30°C (86°F). A tally on paper or in your phone notes is enough.

What is the limit for a Zepbound KwikPen once it is opened?

The label lists three limits for an opened multi-dose vial or KwikPen: 30 total days at room temperature, 30 days after first use, or four weekly doses. Discard at whichever comes first, even if medicine remains.

Does the Zepbound label say anything about freezing?

Yes. The label says not to freeze Zepbound and not to use medicine that has frozen. It also says to protect it from heat and light. Ask your pharmacist if you are unsure whether a pen froze.

A logbook for where the last one went
